HOSPITALITY RESERVATION FORM

The Hospitality Reservation Form is green and is used to register for the following items:

• Seminars: Handling Seminar with Laurin Howard; First Aid Seminar with Dr. Kelly Tart, DVM; Conformation Seminar with Roger Orton.
• Welcome Party
• Eye Clinic: CERF & Glaucoma
• Awards Banquet
• Field Lunches (Friday/Saturday) & Field Dinner
• Dog Food Orders
• Catalogs
• RV Parking

Hospitality order forms are due by April 25, 1997. Make checks payable to FCRSA 97.

MERCHANDISE ORDER FORM

The Merchandise Order Form is white and is used to register for the following items:

• T-shirts
• Sweatshirts
• Jackets
• Caps
• Bath towels
• Travel Mugs

Merchandise descriptions are available online, and forms are due by April 12, 1997. Make checks payable to FCRSA 97.
